You must have the Official Ownership Registration form as shown to the side to register your dog on this site. It must be a pre-printed form from our office. You do not have to mail, fax or email this form after you apply online. Keep this form for your files.
If you have a Certificate of registration or a Pet Records of America registration application, you must mail the forms or fax them to 479-299-4417 or 501-325-7347. We do apologize for any inconvenience. If you have any questions please contact our office at 479-299-4418. Thank you.

If we have to reprint the certificate because of this there will be a $10.00 reprint fee. - The Premium Photo package requires a photo of your dog.
